We want you to be as comfortable as possible for your first office visit and give you an idea of what to expect. Every first visit we conduct a full diagnostic of your bodies current state of wellness. Using a combination of a personal interview with Dr. Jeremy Boethin, x-rays (as needed), and computerized techniques, we are able to get a good idea on what is needed to either address a current issue, move to the next level of health, or begin a plan to help your body achieve the best wellness possible.
When you come in for your first visit we will need to collect some information from you. You can fill out this information in our office (please allow about 10 minutes) or you can download/print/ and fill out. Please find the link below.
Lifestyle Chiropractic is conveniently located near the Old Mill District on Scalehouse Loop. Parking is in the back and the picture show what the building looks like from the parking lot.
You will find our office on the second floor of the Evergreen building. When you enter Lifestyle Chiropractic you will be greeted by Kelly our receptionist and she can help you fill out paperwork, help you with your insurance information, and get you all signed in while you wait for your appointment.
On your first visit, Dr. Jeremy Boethin will do a complete examination using modern techniques to get a complete picture of the overall health of your nervous system. This way, Dr. Boethin can make proper recommendations and provide you with good expectations from treatment.
X-rays are typically done during your first office visit. X-rays provide good information on the overall condition of your spine. X-Rays combined with other modern evaluation techniques helps Dr. Boethin get an accurate snapshot of your overall health and allows him to make the right recommendations for treatment.
You will not likely receive an adjustment until after the x-rays and other evaluation tools are reviewed and a proper recommendation and treatement options are reviewed with you.Thank you very much for your trust and consideration. Believe in miracles, we do!
